So one of your dwarves died trying to get a drink when there's no alcohol, or fishing, or hunting. Maybe you missed it, but if you get this error message, it means that if you don't take action your dwarves will likely die in droves soon. Yes, this is a sign of your apocalypse.

Cause

This is usually caused by dwarves dying in carp/sea lamprey/etc infested waters, then, if you have a finished goods stockpile, your dwarves will try to retrieve the dead dwarf's inventory and place it there. the only problem is that there's still aggressive wildlife there, so when they try to grab for it, they generate this error message. Sometimes they get pulled into the water and murdered, too, making this a big problem.

Solution

Simply forbid the items, or take out whatever killed the initial dwarf.

Or

Watch your dwarves die trying to grab the increasing pile of inventories. This is a great way to deal with floods of immigrants. Or to lose.
