The mother mined a cave wall which caused a collapse. She was sucked down and fell the whole 1 Z level. The mother, with a bruised arm went to rest it off leaving her baby (which she most likely landed on) halfway up the staircase with a broke leg. This poor baby frantically crawled after her, at least that's what it looked like. The baby reached the top of the staircase and promptly took a took a shortcut that involved an open space and a number of injuries. This is the resulting screen-shot.
