A hole to the entrance of the fortress with a wooden wall and watchtowers with marksdwarves stationed in them 24/7 or taking shifts. One entrance is for caravans and thusly has weapon traps. The other entrance is in case I need more time to prepare, the thinner entrance allows more time for soldiers to arrive. This is very effective against ambushes and sieges alike.
