From what I can gather hell in DF seems to just be some kind of cavern containing a bunch of mega-monsters rather then the supernatural "Demon" creatures that usually come to mind; very similar to "The Nether" in Minecraft almost.

For the most part, demons are similar to normal monsters. However, they are closer to being something of mega-forgotten beasts: usually have special attack (fire, poison, syndromes) and are mutated from what is "normal" creatures. Also, once one pierces into the cavern, they start to spawn in the dozens. To compare to Nether: imagine building a portal, going through it, and finding yourself surrounded by 50 ghasts that are able to burn, poison, and rip you to shreds before you get close. And once the portal is there, they follow you to the surface, until they spawn kill you for all of eternity.
