This means that a civilian dwarf had line-of-sight to a hostile creature. It does not matter whether the creature can reach the dwarf.
This message can be your first warning of a rampaging cougar, or foretell your doom in the form "Dwarf cancels Fish: Interrupted by carp." It would be a good idea to find the dwarf in question and investigate, before all of your dwarves start dying because one dwarf died, then another died trying to get his inventory, then another died...
