This means that a civilian dwarf had line-of-sight to a hostile creature. It does not matter whether the creature can reach the dwarf.

This message can be your first warning of a rampaging cougar, or foretell your doom in the form "Dwarf cancels Fish: Interrupted by carp." It would be a good idea to find the dwarf in question and investigate.
