Underworld Spires are huge slade structures built in times before time. Demons that have been brought into the world by deities will build these sites as a bridge between the mortal world and the Underworld and rule from their spire. Goblin sites very often center around an underworld spire, the site of which serves as the capital.
Underworld spires contain a purple staircase simply named as the "underworld portal". Leading to it are several levels of long, winding tunnels with what appear to be prison cells lining the walls, and deeper down, pits filled with weapons and various crafts. The very lowest levels indeed open up directly into Hell, with several demons below.
